Transforming Health Outcomes for children in Foster Care: Lessons from the FCCOE Model
Children entering foster care deserve to receive healthcare services delivered by providers who understand them. This includes an understanding of the trauma related to the removal from their family of origin. While many providers serve children in foster care, typically only a small number have significant experience caring for this population. To build expertise in this area, Coordinated Care’s AHCC plan through partnerships with local healthcare providers has developed a Foster Care Center of Excellence (FCCOE) model to provide expedited, quality, trauma-informed care. The FCCOE model is grounded in AAP standards of care for serving children in foster care coupled with experience in merging Medicaid Managed Care and the child welfare system. This presentation will provide an in depth look at the FFCOE model and detail a healing centered approach to medical and behavioral health for child in and who have experienced foster care.
