Navigating Apple Health Core Connections

In this session, we will discuss Apple Health Core Connections and how to navigate it’s benefits. Apple Health Core Connections is the single source Medicaid plan that covers the majority of children and youth in DCYF’s placement and care authority, including young adults in Extended Foster Care, our alumni population, and youth who have found permanency through adoption or guardianship.
